Please submit this permission/emergency contact form for your youth to participate in the upcoming CityReach overnight program. 

Your 8th-12th grader will arrive in Boston between 7and 7:30pm on Friday evening March 20th and the program will conclude in Boston on Saturday March 21st by 2:15. 

We have 10 slots total and adults are welcome to attend.
if you are interested in participating or being a drop-off or pick-up driver, please indicate that on the form as well.
All fees for this event have been covered by the Westminster SEEK program.

Once you or your young person are signed up below, you will receive an in-depth information packet by email with details about trip expectations and how to prep and pack.

I will provide the RE Field Trip Leaders with any medical information, special diet information, allergies and medication (e.g., inhalers, and including over-the-counter medication) that my child needs with written instructions. 

If my child requires medical attention during this event, I understand that all efforts will be made to reach me. If I can't be reached, I authorize the chaperones to obtain medical services for my youth.  I will list any medical issues or allergies below.

My youth has my permission to attend the Westminster Unitarian Church off-site CityReach overnight service trip with designated adult drivers and chaperones.
